Add a case

Allergy status should be documented for every patient, and drug allergies should include drug/allergen name, reaction, severity, timing and source. Information must be consistent across records and clearly communicated at discharge.

Patient names, NHS numbers, hospital numbers, dates of birth, addresses, telephone numbers or any other direct identifier.
