Who should run this audit?

Doctors, IMTs, registrars, QI fellows, pharmacists, medicines safety leads, ward nurses and clinical governance teams are all well placed to run this audit. Include a named project lead, a consultant supervisor and a pharmacy or medicines safety lead where possible.

What permissions do I need?

Most trusts treat this as clinical audit / service evaluation and do not require ethics approval, but you must register the project with your local clinical audit or governance team before starting. Use only pseudonymised identifiers — do not enter NHS numbers, hospital numbers, names or dates of birth.

How do I sample?

Use consecutive adult (≥ 16y) inpatient episodes over a defined period (e.g. one week per ward). A sample of 30–50 per ward gives a reasonable point estimate; 100+ across the trust supports comparative analysis and ward-level breakdowns.

What does the tool measure?

Whether allergy status, drug allergy details, reaction, severity and source are documented accurately and consistently across admission records, prescribing systems and discharge or transfer communication, in line with NICE CG183 and NICE QS97.

The dashboard updates in real time. Use Findings to interpret results, Improvement to plan actions and PDSA cycles, and Export centre to generate editable Word, PowerPoint, Excel and ARCP evidence outputs.

No. Authentication is handled at the main Lazomis QI website. Inside the tool you will already be in your authenticated workspace, or in demo mode with clearly labelled sample data.
